COVID Update from Ocracoke Health Center

Phase 1B, Group 1, 2, and 3 INCLUDES Individuals 65 years of age & older, and frontline essential workers.

**Ocracoke Health Center is currently still vaccinating 75 years and older ONLY. **

Ocracoke Health Center is beginning pre-registration and scheduling for all of Phase 1B, which include anyone 65 years or older, (modified by CDC recommendation) and health care workers and frontline essential workers of any age.

(* The CDC defines frontline essential workers as first responders (e.g., firefighters and police officers), corrections officers, food and agricultural workers, U.S. Postal Service workers, manufacturing workers, grocery store workers, public transit workers, and those who work in the education sector – teachers and support staff members – as well as child care workers.)

If you fit the description above, please fill out and submit the form below and a call center member will call you back with a vaccine appointment time and date as vaccine is available. If you respond yes on the form and do not have a date of birth that indicates you are 65 years of age or older or fit into one of the qualifying groups defined above, YOUR FORM SUBMISSION WILL BE DELETED.

Estamos comenzando la preinscripción y la programación para toda la Fase 1B, que incluye a cualquier persona de 65 años o más (modificada por la recomendación de los CDC) y trabajadores de atención médica y trabajadores esenciales de primera línea de cualquier edad.

(* El CDC define a los trabajadores esenciale primera línea como los primeros en responder (por ejemplo, bomberos y oficiales de policía), oficiales de correccionales, trabajadores agrícolas y de alimentos, trabajadores del Servicio Postal de EE. UU., Trabajadores de fabricación, trabajadores de supermercados, trabajadores de transporte público y aquellos que trabajan en el sector educativo (maestros y miembros del personal de apoyo), así como trabajadores de cuidado infantil.)

Si se ajusta a la descripción anterior, complete y envíe el formulario a continuación y un miembro del centro de llamadas le devolverá la llamada con la fecha y hora de la cita para la vacuna, ya que la vacuna está disponible. Si responde afirmativamente a la siguiente pregunta y no tiene una fecha de nacimiento que indique que tiene 65 años de edad o más o que encaja en uno de los grupos de calificación definidos anteriormente, SU ENVÍO DE FORMULARIO SERÁ ELIMINADO.
